Who should avoid low-dose doxepin or need extra review?

Short answer

Doxepin is not appropriate for everyone. People with untreated narrow-angle glaucoma, severe trouble emptying the bladder, or certain drug interactions should not use it. Pregnancy, breastfeeding, sleep apnea, liver disease, and mood concerns need review. A licensed physician in your state decides whether treatment is appropriate.

Tell your physician if you have ever had an allergic reaction to doxepin. Untreated narrow-angle glaucoma and severe urinary retention are reasons not to use low-dose doxepin. Recent or current use of an MAOI medicine is another important reason to avoid it.

Share any history of sleep apnea, breathing problems, liver disease, bipolar disorder, depression, or thoughts of self-harm. Doxepin is generally not recommended for people with severe sleep apnea. Older adults may need a lower starting dose because of increased sensitivity to its effects.

Tell your physician if you are pregnant, planning pregnancy, or breastfeeding. Breastfeeding is not recommended during doxepin treatment because it can pose risks to the infant. Discuss an alternative care plan rather than making medication changes yourself.
