Can enclomiphene preserve fertility while treating low testosterone?

Short answer

Enclomiphene may support testosterone production without suppressing sperm production in the way testosterone replacement can. Results vary. It does not guarantee preserved fertility or pregnancy. If having children matters to you, discuss semen testing and a fertility evaluation with your physician before starting any treatment for low testosterone.

Fertility goals are an important part of choosing treatment for low testosterone. Testosterone replacement can suppress the brain's signals to the testes and reduce sperm production. Enclomiphene acts on those signals rather than directly supplying testosterone, so it may be considered for some men who want to preserve sperm production. Results vary.

That does not mean enclomiphene is a proven way to achieve pregnancy or a guarantee that fertility will stay unchanged. A testosterone blood test cannot tell you whether your sperm count or sperm quality is normal. Semen testing provides different information and may be useful before and during care.
