Who should not start a sleep test, device, or medicine without further evaluation?

Short answer

People with complex medical conditions, severe daytime sleepiness, or symptoms beyond routine snoring or insomnia may need further evaluation before starting sleep care. Home testing and Auto-CPAP are not suitable for everyone. Dental problems may limit oral appliance use. A licensed clinician must review your specific risks before recommending treatment.

Some sleep concerns need more evaluation than an online visit or home test can provide. Tell your physician about major heart or lung disease, muscle or nerve conditions, stroke, long-term opioid use, or suspected breathing problems other than obstructive sleep apnea. These factors may change which test or treatment is appropriate.

An oral appliance may not be suitable if you have untreated gum disease, loose teeth, or certain jaw problems. A licensed dentist in your state must assess your dental health. Do not use an over-the-counter mouthpiece as a substitute for prescribed sleep apnea care.

Before insomnia medication, disclose pregnancy, breastfeeding, breathing disorders, liver or kidney problems, falls, and any history of substance use problems. Bring a full medicine and supplement list. These details do not always rule out care, but they can affect the choice. Severe sleepiness needs prompt medical attention, especially if you drive or operate machinery. Do not begin, borrow, or share a sleep medicine or device without proper guidance.
